- Contact our office to schedule a review of your records to determine if you may be a candidate for one of our procedures. Provide copies of your most recent medical and imaging records (we recommend not sending original documents as they cannot be returned). Allow us to review your records at no cost.
- Patients that are suitable candidates will receive a call from a care counselor to schedule their one-time complimentary consultation with one of our highly qualified physicians.
- Consultations are scheduled through video call (via Zoom Video Conference), phone call or in-person appointment at 9777 Wilshire Boulevard, Suite 1004, Beverly Hills, California 90212.

HIPAA Form
The HIPAA form must have been previously signed submitted to one of our Patient Counselors.
Consultation
The consultation will be free in all situations.
Personal History
A letter detailing your personal history must have been previously submitted.
Private Entity
Please be aware that we are a private entity and do not accept insurance.
Waiting Times
Take into consideration that the waiting times for our in-person and online consultations may range between 2 to 3 months due to high demand.
Priority Consultation
If you wish to have a priority consultation, you should send all the aforementioned documents.
What Is Necessary
We understand that in some cases collecting the required information takes more time, for this reason, you can send what you have and you will be able to have a free evaluation which will help determine if you have what is necessary for the consultation.
